Our Eligibilty Requirements​

You must bring your eligibility documents with you to every visit at the clinic If you do not have all of your documents, you will not be seen and will have to call to make another appointment. You must have at every visit a photo ID, proof of income, and proof of address. We are unable to see patients who are above 150% of the Federal Poverty Level


  1. Photo Id
    Government-issued ID OR Passport OR School ID
  2. Proof of Income
    A month's worth of consecutive paystubs from within 30 days of your appointment from you (and your spouse, if applicable) OR If you and your spouse have no income a notarized letter of support from within the past three months from someone who is helping provide food and shelter OR Your state benefits or SNAP action letter from within the past year
  3. Proof of Address
    Cable bill from within one month of your appointment OR Utilities bill from within one month of your appointment OR Cell-phone bill from within one month of your appointment
  4. Proof of Income: Green Card Holders
    If you have been in the U.S. on a Green Card for less than five years, you need to bring your sponsor's information for the first appointment: Sponsor's most-recent income tax return and three months of their paystubs, spouses too AND A notarized letter of support for the patient OR Paystubs of the patient and spouse AND Proof of address
  5. Eligibility Disqualifications
    * Not having all of the required documentation * Being over income requirements (150% of FPL) * Having insurance (including Medicare or Medicaid) * Not living within Prince William County, Manassas, or Manassas Park * Being in the U.S. on a visa * Behaving inappropriately in the clinic or to staff, volunteers, or other patients.

We are fortunate to have partnerships with local hospitals, health departments, and community providers to help us fulfill our mission to care for our community. Please understand that while we partner with many community health providers, we are an independent organization distinct from all other providers and hospital systems. 

If you have been referred to the PWAFC, we are excited to see you as a patient. Please call and make an appointment to see one of our providers or visit us during our walk-in clinics. You must bring your proof of address -- showing that you reside in Prince William County, Manassas, or Manassas Park -- and your proof of family income that is dated within 30 days of your appointment at the clinic.
